
First Visit
Your first visit will be a consultation with Dr. Carey which includes an assessment of your case, including a review of information provided by your dentist. If necessary, diagnostic x-rays will be taken so that your situation can be thoroughly evaluated, and the ideal treatment plan can be determined. Dr. Carey will discuss your diagnosis, explain procedures and sedation options, and create a treatment plan that best fits your individual needs. You will also meet with our Treatment Plan Coordinator who will discuss your treatment costs, insurance benefits, and payment options. It is at this time your surgery will be scheduled.
Please assist us by providing the following information at the time of your consultation:
- A valid form of photo identification.
- Necessary documentation of your medical and/or dental insurance.
- Payment for any applicable co-pays or fees.
- Please fill out the Patient Registration Formonline before your visit or arrive 20 minutes prior to your appointment time to fill it out in the office.
IMPORTANT: All patients under the age of 19 years of age must be accompanied by a parent or guardian at the consultation visit.

Please note that we are not a contracted provider with any Medicare or Medicaid insurance plans. If you have a Medicare or Medicaid plan, they will not provide payment for services rendered in our office and payment will be your responsibility.

Financial
For your convenience we accept Debit Cards, Credit Cards, HSA/FSA Cards, cash, and checks. We also accept Care Credit. Payment is due at the time service is rendered. If you have questions regarding your account, please contact us.
Most insurance companies will respond to claims within four to six weeks. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. Contact our office with any questions about your statement.
